Please allow at least 48 hours when contacting a doctor. Please include your Last, First, Middle names, your patient number (if known), your date of birth (mm/dd/yyyy), day time telephone number in all email/message. If requesting a refill, please please include the Rx#, the pharmacy's name, pharmacy's address and the pharmacy's telephone number. In some cases such as prescription refill, the physician will have the nurse call you to inform you that the prescription was faxed to the clinic.
